My name is Tiffani Phillips. I have been in the senior living/healthcare arena for many years. I am a Licensed Social Worker and love working with Seniors and their families. I have established my business and reputation by knowing the value of communication, accountability, responsibility, resourcefulness, compassion and follow through. I commit the same values to you to entrust to me and A Place For Mom.
A Place For Mom is a national referral service for families to explore services that assists you, your team, patients and their 'senior living' including Memory Care, Assisted Living, Independent Living, Respite Stays and Private Duty Health. A free service to you, your patients and families.
A few key indicators your patient, a family member, neighbor, or friend may be ready for A Place For Mom are, but limited to over 55, frequent falls, repeat hospital readmissions, dementia, no longer able to drive loss of a spouse/ significant other, chronic disease socialization needs, medication management, etc.
